Understanding the 4 NHS discharge pathways
The NHS uses four standardised discharge pathways to route patients to the right level of post-discharge support. We are a Pathway 1 provider.
Pathway 0
Simple discharge
Patient can go home with minimal or no support. No ongoing care input required.
Pathway 1
Discharge with support
Patient returns home with short-term support from health and/or social care services. This is where we operate - providing safe, nurse-led homecare. Response times are subject to referral complexity and available capacity.

Patient requires bed-based short-term rehabilitation in a community hospital or care home setting.

Patient requires ongoing residential or nursing home care.
