From a few hours a week to full live-in care, we provide flexible, nurse-led support across Hampshire and the surrounding area.

A dedicated carer lives in your home, providing round-the-clock support with personal care, medication, meals, companionship, and overnight monitoring.

How we support your discharge home
We provide Pathway 1 discharge support, working with discharge coordinators, GPs, and community nurses across Hampshire.
Referral received
We acknowledge every referral within 2 hours, 24 hours a day, 7 days a week. Call 0330 0438 445 or use our online referral form.
Nurse-led assessment
Our registered manager or a qualified nurse conducts a comprehensive assessment - in hospital, at home, or via secure video - to understand the individual's needs.
Carer matched
We carefully match the right carer by clinical need, gender preference, language, personality, and location. No rushed placements.
Care begins
Care commences as quickly as possible following referral - typically within 24–48 hours, subject to referral complexity and available capacity. Full handover documentation is shared with the discharge team, GP, and community nurses.
